چکیده
THE Bispectral Index (BIS) monitor (BIS® monitor; Aspect Medical System, Inc., Newton, MA) integrates electroencephalographic data via a proprietary algorithm based on empirical data into a nominal 100 to 0 scale, in which 100 represents the awake state and 0 represents the isoelectric electroencephalogram. The BIS® monitor is highly capable of correctly predicting both the loss and recovery of consciousness and has been reported to be more reliable in assessing the level of sedation and hypnosis during surgery than other available processed electroencephalogram algorithms. It also has been used clinically to guide anesthetic drug administration and to quantify the pharmacokinetic and pharmacodynamic action of anesthetic drugs in the laboratory setting. We report a case of a rapid decrease in the BIS score during total intravenous anesthesia using propofol after cross-clamping the descending thoracic aorta for elective thoracic aortic aneurysm repair and suggest that the BIS® monitor provides useful information on altered propofol pharmacokinetics after aortic cross-clamping.

Influence of the descending thoracic aortic cross clamping on bispectral index value and plasma propofol concentration in humans.
BACKGROUND In this study, the authors investigated changes in Bispectral Index (BIS) values and plasma propofol concentrations (Cp) after aortic cross clamping in the descending thoracic aortic aneurysm repair surgery during propofol anesthesia. METHODS Prospectively, in 10 patients undergoing thoracic aortic surgery during total intravenous anesthesia with propofol, BIS values were recorded ...
متن کاملCross-clamping of the descending thoracic aorta leads to the asymmetrical distribution of propofol during cardiopulmonary bypass surgery
BACKGROUND We hypothesized that cross-clamping of the descending thoracic aorta (CcDTA) would result in significant changes in plasma propofol concentrations (Cp) proximal and distal to the cross-clamp. We investigated the effect of CcDTA on Cp centrally and distally, including the pulmonary artery and the cardiopulmonary bypass (CPB) cannula. METHODS The bispectral index (BIS) was recorded d...

متن کاملBispectral index profile during carotid cross clamping.
This study aimed at investigating the Bispectral Index (BIS) profile during carotid cross clamping (CXC). The study involved a pilot group of 10 patients undergoing routine carotid endarterectomy with shunt insertion under total intravenous anesthesia, and a study group of 26 additional patients. In all patients, rates of propofol and remifentanil providing a steady-state level of hypnosis (BIS...
متن کاملEffectiveness of assistance circulations for distal circulatory support during cross-clamping of the descending thoracic aorta.
In order to find the ideal distal circulatory support during cross-clamping of the descending thoracic aorta, the author compared the effects of simple shunting (SS) and centrifugal pump (CP) on hemodynamics and metabolisms in mongrel dogs. In group I (control), the aorta was cross-clamped for two hours without SS or CP; in group II (SS), a temporary shunt was placed between the left common car...
